CCMAP Application ProcessReceiving medication assistance starts here

The process to apply for CCMAP assistance is simple.

Applicants may be eligible if they do not have prescription drug coverage and have a family income below 250 percent of the Federal Poverty Guidelines.

Individuals can fill out the application available on this website and mail or deliver it in person to CCMAP.

The application will be briefly reviewed by CCMAP staff and an appointment time will be set for the patient.

The patient will need to bring in all financial documents, proof of residency and all prescriptions to this appointment to complete enrollment in the program. CCMAP staff will review all of the details and requirements of the program with the patient during this interview as well.

Upon full enrollment, the applicant will be eligible to receive medications. The applicant will also be provided with the necessary paperwork for their physician to sign.
